FEES AND FORMS

Below are details of fees for services rendered at Affirmative Counseling and Psychological Services, LLC as of September 1st, 2019.

INDIVIDUAL THERAPY FEES

  • Individual Intake Appointment: 60 minutes - $150

  • Individual Ongoing Appointment: 45-50 minutes- $125

RELATIONSHIP & COUPLES THERAPY FEES

  • Emotion Focused Therapy Intake Package - $500 (includes following)

    • One Initial Couples assessment appointment together: 75 minute

    • One Individual assessment appointment with each partner​: 45-50 minutes

  • Couples Therapy Ongoing Appointment: 60 minutes - $150

  • Additional Individual appointments as requested/needed during ongoing relationship therapy- $125

GROUP THERAPY FEES

  • Group Therapy Screening Fee: 30 minutes - $50

  • Group Therapy Ongoing Rates: $60+ depending on group type and length

ADDITIONAL FEES

  • Consultation Fees: $125/hour (Please contact for Flat Rate All Inclusive Fee)

  • Supervision Fees: $135/hour (for mental health professionals)

  • Fees for Writing Official Letters (e.g. Providing Referral Letter to physician or outside therapist, Absentee letters for work or school): $75 flat rate

  • Late cancellation or no show fees: Full fee for scheduled service (see fees for services above)

  • Workshop/Presentation Fees: $200/hour (including both preparation and delivery of workshop or presentation). Please contact for Flat Rate All Inclusive Fee

DO YOU OFFER SLIDING SCALE RATES?

Yes, on a case by case basis. I believe therapy is an investment of time, energy, and resources by me and my clients and I enjoy talking with prospective clients about ways to balance these. When you call to schedule your appointment you will be provided information about fees for service and reminded that I do not accept insurance. I do offer a discount for those who want to pay for multiple sessions in advance. I believe my rates are competitive but I am open to hearing about your unique situation and will make accommodations where possible. Also, during your course of therapy, if your financial situations changes and therapy becomes unaffordable, please inquire about ways we can manage fees for service. If I can accommodate your needs, I will.

DO YOU ACCEPT INSURANCE?

I currently do not directly participate with insurance plans. There are a number of reasons why this is the case and I am happy to have a conversation with you about fees for service. However, I will assist you if you wish to use your insurance by giving you receipts to submit directly to your insurance provider. Many insurance companies will pay for a portion of outpatient mental health services. You should check with your insurance company representative to find out specific requirements and limitations of this coverage. Payments for services received through Affirmative Counseling and Psychological Services, LLC are ultimately your responsibility.

PAYMENT TYPES ACCEPTED

At Affirmative Counseling and Psychological Services, LLC we accept cash and check as well as all major credit cards (AMEX, Visa, Mastercard). Payment is expected at time of service.

CANCELLATION POLICY

If you find that you need to cancel an appointment, please give as much notice as possible so that I can schedule others that may be waiting for services. You will be personally charged for your appointment if not canceled at least 24 hours in advance other than for emergency reasons. If more than two appointments are cancelled within 24 hours future sessions will need to be paid 48 hours in advance via PayPal payment. If you no show for an intake appointment or cancel your intake appointment within 24 hours of your appointment time you will not be able to reschedule without first paying the full fee for your missed appointment and your rescheduled appointment.
